Now that several states have passed bills that effectively ban abortion after a fetal heartbeat can be detected, the new laws are raising a lot of questions about early pregnancy and miscarriage treatments. So when do women typically realize that they are pregnant? And how often are pregnancies unplanned? It sounds odd, but doctors measure the beginning of a pregnancy as being the first day of your last period. The reason doctors still use the last menstrual cycle as a benchmark is because it is difficult to know exactly when the sperm fertilized the egg.
So when doctors say a woman is six weeks pregnant, it typically means the embryo started developing about four weeks ago. The heart, which can be seen flickering on an ultrasound, is still maturing and cannot be heard until several weeks later. Perhaps this is the simplest way to say it: Six weeks pregnant is two weeks after a woman misses her period. Georgia, Kentucky, Mississippi and Ohio have this year passed so-called heartbeat bills, which effectively prohibit abortion after six weeks, and other states are poised to follow in their footsteps.
According to the American College of Obstetricians and Gynecologists, one of the biggest concerns of their members right now is about how to interpret the scope and application of these laws in their practice. In some instances, that extends beyond the delivery of abortion care to questions about miscarriage treatments like dilation and curettage, which removes tissue from inside of the uterus.
Women are also wondering how miscarriage will be interpreted under the law. For now, abortion is still legal because these laws have not yet taken effect.
Whilst you may get a positive pregnancy test before your period is even due it is a couple of weeks before the pregnancy is big enough to be seen on scan. Somewhere between 4 and 5 weeks, we can sometimes see a small black fluid area in the uterus but we cannot confirm it is a pregnancy sac until we can see something growing inside it.

For the purposes of research and surveillance, the best obstetric estimate, rather than estimates based on the last menstrual period alone, should be used as the measure for gestational age. At six weeks, some doctors perform a sonogram, also called an ultrasound, to determine the health or viability of the developing embryo. This is generally the earliest that anything is visible on an ultrasound, although at this point it will not look much like a baby, since development is still in the early stages. At this early in a pregnancy, it is difficult to see anything at all using a standard external ultrasound machine.
Often, if a six-week sonogram is performed at all, it will be using a vaginal ultrasound machine, which consists of a wand-shaped device that is inserted into the vagina. A six-week ultrasound can determine the location of the embryo and ascertain that it is in the correct place in the uterus. If the pregnancy is an ectopic pregnancy, with the embryo implanted outside the uterus in the Fallopian tube, this can be determined based on the blood flow patterns seen via ultrasound.
The fetal heartbeat is often detectable at a six-week ultrasound. The normal fetal heartbeat at six weeks is about 90 to beats per minute. Detecting a heartbeat at this stage indicates that the pregnancy will probably continue and not end in miscarriage, although this is not absolutely guaranteed. If the sonographer cannot detect a heartbeat, the pregnant woman will generally be advised to come back in another week or two to check again, since sometimes a heartbeat that is undetectable at six weeks may be stronger and more noticeable at seven weeks or more.
The fetal pole is the basic overall shape of the embryo, which can be seen via sonogram at around the six-week mark. The fetal pole resembles a bean in shape, which the technician can look at and determine the head and rump ends of the embryo. Seeing the fetal pole allows the sonographer to measure the size of the embryo.

In early pregnancy, especially before 11 weeks, it is usual to have a trans-vaginal internal scan , where a probe is placed in the vagina. This gives the clearest and most accurate picture in early pregnancy. The person doing the scan spreads a special gel on your lower abdomen below your belly button and above the line of pubic hair. He or she then moves the scanner over the gel, sometimes pressing down, until the uterus womb and pregnancy can be seen.
That may give some information about your pregnancy, but it is less clear than an internal scan and that could possibly delay diagnosis. There is no evidence that having a vaginal or an abdominal scan will cause a miscarriage or harm your baby. If you bleed after a vaginal scan, it will most likely be because there was already blood pooled higher in the vagina and the probe dislodged it.
All too often, a scan at 6 weeks shows very little or nothing, even in a perfectly developing pregnancy, whereas waiting a week or 10 days will make the findings much clearer. Most pregnant women are referred for their first routine or booking ultrasound scan somewhere between 11 and 14 weeks of pregnancy.

An ultrasound is a highly accurate means of dating a pregnancy. In a normal pregnancy, ultrasound can provide an estimate of the gestational age to within five to seven days of accuracy. In the first trimester, doctors usually use a transvaginal rather than abdominal ultrasound. The transvaginal ultrasound provides the most accurate information, given that the early developing gestational sac and fetal pole are extremely tiny at this point.
A transvaginal ultrasound can get closer to the developing pregnancy. In a transvaginal ultrasound, the doctor or technician inserts a thin probe into the vagina. During the ultrasound, they take a series of measurements, including the size of the gestational sac, the size of the yolk sac, the length of the fetal pole, and the heart rate.
In an abdominal ultrasound, the woman will be asked to come to the scan with a full bladder, because this positions the uterus in a way that makes it easier to obtain measurements. The doctor or technician then spreads a gel over the lower abdomen and uses a transceiver to take measurements from a number of angles. These are averages for when specific early pregnancy developmental points become detectable by transvaginal ultrasound. Generally, abdominal ultrasound is less sensitive and may not detect these milestones until a week or later in the pregnancy.
Remember that any uncertainty about the date of ovulation could affect what the ultrasound will show at any point in early pregnancy.
